We accept purchase orders for institutional orders only and only from organizations within the United States. International clients please use our convenient online store or contact us for wire transfer instructions.

1. Please include buyer name, organization name, phone number, email address, billing and shipping address, method of shipment, and also the quantity of DVD(s) to be ordered.

2. Optional: If you wish to purchase a lifetime Streaming License for the DVD, please add $145.00.

3. If you are located in the State of Florida, please add 7% sales tax to your total order.

4. Specify shipping method and include estimated shipping charges. See below for shipping charges on a single DVD order, or contact us for shipping rates on a larger quantity order.
